About the Opportunity
We are currently looking for a Discharge Planner to join our inter-disciplinary team of skilled professionals. Reporting to the Clinical Manager, the incumbent is responsible to provide consultative advice to patients, families, physicians and others to facilitate individualized, safe, and timely transitions home or to the next level of care. The Discharge Planner assures proper placement at the appropriate time to ensure quality and continuity of patient care.
This is a Non-Union position, working 5 to 8-hour shifts, primarily Monday to Friday.

More About You
Basic Requirements
- Current certificate of competence from the College of Nurses of Ontario (CNO) or current certificate of registration in good standing from the Ontario College of Social Workers and Social Service Workers, or other appropriate professional body.
- Demonstrates a comprehensive knowledge of community services, agencies and placement facilities.
- A minimum of three years’ experience in the community health field is preferred.
- Demonstrates excellent communication skills and collaboration when interacting with patients, family members and other members of the care team.
- Sound knowledge of discharge planning principles and a thorough working knowledge of their application in a chronic care setting.
- Demonstrates a knowledge of financial aid available from Federal, Provincial and Municipal programs.
- Respect for individuals of diverse cultures and demonstration of culturally sensitive practice.
- Demonstrated satisfactory attendance levels, work performance, disciplinary history, and positive work references, as well as effectiveness working with minimal supervision, with proven reliability.
- Demonstrated knowledge of various computer applications and software related to healthcare programs.
